CONTESTANT FORMS

STEP 1 | Download Official Handbook

STEP 2 | Items due by May 1st​

1. Submit “Become a Contestant” Online Registration Form

You will need to provide contact info for you and your parent/guardian as well as some basic biographical information about yourself.

2. Submit “All About Me” Online Form

You will need to prepare thoughtful answers for this required form which is used as a resume during panel interviews. Refer to the Official Handbook linked above for guidance, tips & complete instructions.

3. Submit Registration Fee

You are required to pay Registration Fee of $50 which is due on or before May 1st. You can also pay in person by appointment with Mr. Scott.

NOTE: $25 EARLY BIRD Discount if received on or before April 1st!

STEP 3 | Items due by May 9th

4. Submit “Official Release of Liability” Form

You and a parent/guardian are required to complete and sign this required online form. Please refer to the last page of the Official Handbook linked above. You may also print, complete and sign hard copy of the form and submit by required deadline.

5. Participate in Contestant Photo Shoot in person at Leonard Photography

If you are unable to attend this scheduled activity, you must make your own arrangements with the photographer for a make-up session BEFORE THIS DEADLINE. Note that an additional sitting fee may be charged by the photographer at your expense. Alternatively, you can submit your most recent school photo in jpg or png format via email to info@MantecaYouthFocus.org

STEP 4 | Item due by May 19th

6. Submit Interview Wardrobe Approval Form

This form requires you to upload photos of the front and back view of your outfit worn for the Personality Interview phase. Refer to the Official Handbook linked above for Wardrobe Guide.

STEP 5 | Item due by June 5th

7. Submit “Volunteer Service” Form

You will need to gather the following information for AT LEAST one but NOT MORE than three organizations for which you have volunteered at some point during the past two years. Refer to the Official Handbook linked above for guidance, tips & complete instructions.

  1. Club/Organization Name
  2. Indicate whether it is a School or Community organization
  3. Years of Service
  4. Name, phone number and email of adult reference from this organization (NOT a family member)
  5. List/Describe activities completed for each organization in the past two years (250 words or less)

8. Submit “Academic Achievement” Form

You will need to gather the following three items in order to complete the required form. Refer to the Official Handbook linked above for guidance, tips & complete instructions.

  1. A thoughtfully-prepared Personal Academic Statement of 500 words or less to enter into the form where indicated.
  2. Transcript/Report Card from the current academic year. Save as PDF file to upload onto the form where indicated.
    1. If you are in grades 3-8, you MUST submit your report card for THIS academic year.
    2. If you are in grades 9-12, you MUST submit your transcripts for your ENTIRE high school career. For MUSD High Schools, you can easily request your transcript online via Parchment (link here).
    3. If you are in college, you must submit your transcripts for your ENTIRE college career.
  3. A SIGNED letter of recommendation from a teacher, counselor or administrator. Save as PDF file to upload onto the form where indicated.

9. Submit “Community Advocacy Speech” Form

You are required to provide the EXACT content of your Community Advocacy Speech (100 words or less)

10. Submit Entry Fee 

You are REQUIRED to pay Entry Fee of $200 due on or before June 1st. (In person by appointment with Mr. Scott)

You may obtain sponsors for this purpose. Based upon the amount of their donation, they may be eligible for show tickets and promotional consideration. Refer to the Official Handbook linked above for Donation Letter and Sponsor Form you can provide to potential sponsors.

NOTE: Additional prizes available for sponsor recruitment above $200. Sponsor recruitment prizes have NO BEARING on the outcome of the competition.

STEP 6 | Items due by June 12th

11. Submit Formal Wear Wardrobe Approval Form

This form requires you to upload photos of the front and back view of your Formal Wear wardrobe worn for the Communicative Skills Phase of Competition. This outfit will also be worn during the final crowning portion of the competition. Refer to Official Handbook for Wardrobe Guide.

12. Submit Stage Arts Form

This form requires you to provide mandatory information in advance so that the technical directors can prepare the materials for the competition show and technical rehearsal.

  1. Brief description of your stage arts presentation.
  2. Title of Work
  3. Original Artist
  4. List of EXACT props, instruments and/or equipment that will be used on-stage.
  5. Front and Back photos of the outfit you will wear for this phase of competition. See Official Handbook for specific rules & regulations related to stage arts wardrobe.
  6. If you plan to use music for your presentation in the Stage Arts phase of the competition, you are REQUIRED to submit audio track EDITED for the 2:00 minute time limit.

THESE FORMS ARE REQUIRED FOR ALL AGE LEVELS
You should begin the process of completing your online application forms early, entering the required information and uploading required support documents and files by the stated deadlines. These will be the official forms used by the judges to score you for the various phases of competition. Pay attention to each section as the rules are specific, any missing information or late information may result in a reduction in your score for the applicable phase of competition. Most sections have word limits and each form has specific deadlines. Let’s dive into what is required for each of these items listed above.
